By filling out this form, I authorize the Foundation to collect my personal information and use some of it to manage donations and the resulting recognition requirements and activities (e.g. sending a tax receipt, managing donor recognition programs, calling for nominations for directorships, sending an invitation to the AGM, etc.).

Please note that you will receive a consolidated receipt in February for all donations in the amount of $10 or more that you have made over the previous tax year.
